BENEFITS-FULL TIME EMPLOYEES

Vacation (Vacation is accrued for employees that are set up and work 30 hours or 36 hours or more per week)

Vacation is accrued on the number of hours you are paid each pay period up to:


After 1 full year full time
Up to 80 hours of vacation

2-4 years full time
Up to 80 hours of vacation

4-9 years full time
Up to 120 hours of vacation

10 years and over
Up to 160 hours of vacation


Vacation should be taken within the following year after available if at all possible.


Vacation Pay Out

If you leave employment for any reason and have been employed for over 1 year, vacation accrual time will be paid out in 80 hours increments or as designated by the facility.


Sick Time (Sick time is accrued for employees that are set up and work 30 hours or 36 hours or more per week)

Sick Time is accrued on the number of hours you are paid each pay period up 80 hour a total 3.69 per pay period equal to 12 days or 96.0 hours per year with a maximum accrual of 1040 hours.

You may be paid on the first day off sick if you have on the books an accrual amount of 96.0 hours or more. If you do not, you must be off 12 scheduled work hours and then you will be paid up to the amount you have accrued. Pay will not go back to the first day you were off sick.

Exception to this rule: if you are hospitalized on the 1st day sick or have a surgical procedure then you may be paid on the firs day if you have sick time available. You must have been employed for 90 days to begin using sick time.

If you leave employment at Three Rivers for any reason, there is no pay out for sick time accrual.


Holiday Time (Holiday time is paid to employees that are set up and work 30 hours or more per week)

You are paid seven (7) holidays at 8 hours per holiday per year at Three Rivers Hospital:

1 Personal Day
New Years
Memorial Day
4th of July
Labor Day
Thanksgiving
Christmas

If you work the holiday, you will receive another day off. You may take holiday 30 days before or 30 days after the holiday. Please use your holiday within the time period.


Life AD & D (Must be set up and work 36 hours or more per week)

100% Employer paid
Two (2) times base salary
Eligible the first of the month following 30 days of employment
All employees working full time (36 hours or more) eligible


HEALTH, VISION, PRESCRIPTION AND DENTAL INSURANCE (Must be set up and work 36 hours or more per week)

For Full time employees (36 hours or more)

After you have been employed full time for 30 days, you will be eligible the first of the month following the 30 days.

If you owe a bill to Three Rivers Hospital and wish to pay this account through payroll deductions you may check with the Business Office and Human Resources and pay in this way.


401 K Program

Three Rivers Hospital has a 401 K program available to employees. Further information is available through Human Resources.
